First Notice of Loss (FNOL) Disclosure

This disclosure explains what happens when ClaimBeacon files a First Notice of Loss on your behalf with your insurer, and what your rights are under the Information Regulator (South Africa) and applicable insurance law.
1. What we collect at the scene
- GPS coordinates and a street address derived from them.
- Time of occurrence and a narrative you write or dictate.
- Photos and short videos you choose to upload (vehicles, scene, documents).
- Witness names, contact details, and statements — submitted by witnesses themselves through a single-use link.
- Other-driver registration, licence number, and contact details where provided.
- Whether any injuries were reported.
2. What the FNOL document contains
The FNOL PDF compiled and stored by ClaimBeacon includes your identity (name, contact, ID number), the policy summary, the insured vehicle, the incident facts above, the witness and other-party data, and a declaration of truthfulness. Every regenerated version is retained — earlier versions are not deleted.
3. Who receives it
The FNOL is shared with: your insurer tenant, your broker tenant (if any), the assigned assessor, and any handler your insurer assigns to the claim. It may be re-disclosed by your insurer as required by law, regulation, or reinsurance arrangement.
4. Automated decisioning
ClaimBeacon's AI suggests a triage path, severity, and fraud-risk score. These outputs are advisory. They do not by themselves approve, reject, or set the value of your claim — that decision is made by an authorised person at your insurer. You may request a human review of any automated output that affected the handling of your claim.
5. Your rights at the FNOL stage
- Truth & correction: You can submit a correction request to your insurer at any time. ClaimBeacon will regenerate the FNOL on staff approval and keep all versions.
- Access: You can download every version of your FNOL from your account.
- Witness withdrawal: A witness may request removal of their statement by emailing privacy@claimbeacon.app. Where the statement is required by law or a live investigation, removal may be deferred.
- Complaint: If you are unhappy with how an FNOL was handled, contact your insurer first; you may then escalate to the Information Regulator (South Africa).
6. Tampering & fraud
ClaimBeacon attaches an immutable audit trail to every FNOL action: who generated, viewed, amended, or regenerated it, when, and from what device. Tampering with or fabricating any part of an FNOL is a criminal offence in most jurisdictions.
